New Athlete Registration
Special Olympics Sydney Upper North Shore Registration period is 1 January to 31 December.

Welcome Letter from Chairman Detailing 2012 Fees (click on this link)
1. Read the 2012 New Athlete Registration Form and welcome Letter (see above for links). Complete all sections of the registration form.
2. Complete the 2012 Athlete Invoice (click on this link)
3. Forward the completed and signed form and invoice with the annual registration fee of $70 ($35 if you are registering after September) plus $30 per term per sport fee to:

If you wish to you can deposit the fees directly into the Special Olympics Sydney Upper North Shore bank account. The account is with St George Bank, account number 552924519 and BSB 332 018. If you do this you MUST email our secretary@souppernorthshore.com so that we can trace your money coming through the account. In this case you should still send the invoice and registration papers by mail.
3. Please remember to circle agree or disagree to media consent on page 2. Please note that if you disagree we will not use any photos or write any articles on the athlete for the Upper North Shore website.
4. The State Registrar will assign a Registration Number and issue a receipt for fees received to the Regional Registrar for distribution to the Applicant.
5. Registration Period is 1 January to 31 December
6. Annual re-registration is required
7. The medical clearance only needs to be given every four years.
Medical Clearance for Athletes With Down Sydndrome
1. Clearance by a registered medical practitioner that the athlete does not have Atlano-Axial instability. The evidence of this can be from when the athlete was younger - it does not have to be recent.
2. Clearance by a registered medical practitioner that there is no current evidence (symptoms) of Atlano-Axial instability. A repeat review is required every 4 years. An x-ray is not required.
3. Clearance by a registered medical practitioner specifically for activities including gymnastics skills eg rolling and trampolining
