Registration
Special Olympics is a year-round sports training and athletic competition program for children and adults with an intellectual disability.
The ultimate goal of Special Olympics is to help persons with an intellectual disability participate as productive and respected members of society at large, by offering them a fair opportunity to develop and demonstrate their skills and talents through sports training and competition, and by increasing the public’s awareness of their capabilities and needs.
Instructions:
1. Read the Application for Participation Form, and complete all sections.
2. Forward the completed and signed forms with the registration fee of $120 to Special Olympics at PO Box 262, Mona Vale 2103. For more information regarding the increase in registration fees for 2010 please read the letter-to-our-athletes-re-2010-fees from our 2009 chairman Greg Simmons.
2. Please remember to circle agree or disagree to media consent on page 2. Please note that if you disagree we will not use any photos or write any articles of the athlete for the Upper North Shore website.
3. The State Registrar will assign a Registration Number and issue a receipt for fees received to the Regional Registrar for distribution to the Applicant.
4. Registration Period is 1 January to 31 December
5. Annual re-registration is required
6. The medical clearance only needs to be given every four years.
